If you are under the age of 18, Indiana law requires minor to provide one of the following:

*Legal Birth Certificate

*Valid State Issued ID or Driver's License

*Valid Passport

*PARENT AND MINORS LAST NAME AND ADDRESS MUST

MATCH ON ID OR WE CANNOT ACCEPT.  NO EXCEPTIONS!

IF YOU CANNOT PROVIDE THEN A BIRTH CERTIFICATE IS REQUIRED

We CANNOT accept the following:

*Medical Card

*Social Security Card

*Hospital Card (birth)

*Work ID

*School ID

At least ONE parent or legal guardian must be present to sign consent form and provide one of the following:

*Valid State Issued ID or Driver's License

*Valid Passport or

*Military ID

If you are legal guardian, you must ALSO provide court documents showing you are legally allowed to sign for minor.  This is required by Indiana Department of Health and Services.
